NDIS Referral Form

Please read the note below before continuing. It’s here to clarify consent, funding access, and how referrals are allocated.

This referral form does not give Lime Rainbow consent to create a service booking, nor does it give us access to the participant’s NDIS funding.
On receiving new referrals, allocations are made as practitioner capacity allows. Please note that to ensure the wellbeing of our team and the quality of our service, Lime Rainbow does maintain a waitlist if required.
Plan information is sought solely to support Lime Rainbow in understanding your service needs and to allocate the most suitable practitioner to support you.
At no time are you obliged to provide a copy of your NDIS plan to any provider if you choose not to.

We review and allocate on Tuesdays and Fridays each week with a response usually that afternoon.

We do not currently have a waitlist for any of our service offerings.
